Towards Safe Blood in Nigeria

Situation: 

The healthcare system in Nigeria has an increasing need for blood transfusion to manage postpartum bleeding, trauma, major surgery, severe anemia in children, sickle cell disease among others. The available blood is not always safe: there is still a risk of transmission of infectious diseases (inadequate testing, bacterial contamination), inadequate hygiene measures, transfusion reactions due to red cell antibodies (no reagents/facilities for testing).

 

Objectives:

To establish the prototype for national training centers for transfusion medicine at Aminu Kano Teaching Hospital providing target group adjusted training for blood donation clinics, blood banks and auditors from across Nigeria. 
To establish a national network for improving the supply of safe blood throughout Nigeria and to support sustainability by regular training and coordinated auditing. 
 

Indicators:
  • The training center at AKTH is established and the curricula are developed
  • 10 participating centers are selected and have been audited and 10 re-audits have been performed. At least 5 re-audits performed with participation of peer-expert auditors, which show measurable improvement in the audit results between the initial and the re-audit in at least 7 of the 10 participating centers (trained staff >10 staff per hospital)
  • 4 trainings for transfusion medicine experts (physicians) and laboratory qualified staff and 1 workshop for hospital administration have been performed (trained n=80) and 2 auditor training workshops have been performed (trained n=30)
     
Measures:

Sustainability:

The changes in praxis have been achieved at AKTH through available resources of the hospital independent of external funding. Part of the training is to show the participants what has been established at AKTH and how this can be achieved with the resources available in Nigeria. 
The integration of the project into the strategic developmental plan of the NBSA and regular follow up audits and supervision of blood banks by the NBSA will strongly support sustainability of the project achievements. At the end of the project, the NBSA and the federal ministry of health and AKTH will be responsible for the training centre and the continuation of training and audits.

Special features:
  • The project is a cross-sectional project in healthcare.
  • The National Blood Service Agency (NBSA), as the Ministry of Health's national authority for ensuring safe blood supply, is an active project partner. The project will be part of the national health strategy.
  • Through the project, the Nigerian project partners establish a self-reinforcing spiral of training and evaluation through feedback audits.

The availability of safe blood is a pre-requisite for almost all medical fields (obstetrics: postpartum hemorrhage; surgery: major interventions; internal medicine: oncology, sickle cell anemia; pediatrics: malaria; etc.). This was achieved by the project partners at the AKTH University Hospital. Now, together with the National Blood Service Agency (NBSA), the Ministry of Health's national authority for ensuring safe blood supply, other centers in the country are being trained to implement the progress achieved at AKTH in their own hospitals. At the same time, auditors are being trained in Nigeria, who, together with the NBSA, maintain the safety of blood supply and transfusions through regular audits.
