Hodgkin lymphoma (HL) is a malignant B-cell lymphoma characterized by alterations in several signaling pathways. Clinical features and response evaluation by positron emission tomography lack sensitivity for individualized treatment guidance. Using circulating tumor DNA sequencing, we were able to describe three biological subtypes of HL with distinct genomic, clinical and tumor microenvironmental (TME) features. Inflammatory immune escape HL is characterized by copy number variations affecting immune escape variants and an inflammatory TME. Virally-driven HL is frequently associated with Epstein-Barr virus and signs of chronic inflammation in the TME. In contrast, Oncogene-driven HL is characterized by recurrent mutations in oncogenic drivers, resulting in a high tumor mutational burden. An independent clinical validation showed meaningful progression-free survival differences between subtypes.
